Absolutely.  An employee write up can follow a simple formula - 
  • Who is involved 
  • What rule or policy was violated 
  • Remediation implemented to forestall future occurrences 
  • Presentation to the employee for review and sign off  

If the employee refuses to sign, indicate so on the write up.
